Before requesting a quote
The application states that a “yes” answer to any of the following makes an applicant ineligible. Please note the timeframes and exceptions.
- Cancer within the past 24 months: diagnosis, treatment, or prescribed medication for cancer, including leukemia, lymphoma, malignant melanoma, or cancer that has spread. Exception: skin cancer in situ.
- HIV/AIDS: treatment or diagnosis of AIDS or AIDS-related complex, or testing positive for HIV. No lookback period is stated.
- Current eligibility or disability: eligible for Medicaid, on Medicare before age 65 due to disability, or currently disabled.
- Current daily assistance: receiving assistance or supervision with bathing, dressing, eating, toileting, or getting into or out of bed; or inability to control bowel or bladder function.
- Current care: receiving home health care services, or confined in a rehabilitation, nursing, or assisted living facility.
- Cognitive or neurological conditions: currently being treated for, or having been diagnosed by a medical professional with, Alzheimer’s disease, dementia, Parkinson’s disease stage 4 or 5, Huntington’s chorea, or cognitive impairment.
- Current treatment for diabetic complications: neuropathy, proliferative retinopathy, kidney disease or failure, renal insufficiency, or kidney dialysis.
- Current treatment for insulin-dependent diabetes together with heart failure.
- Unfinished medical evaluation: within the past 12 months, advised to have tests, treatment, or surgery not yet performed, or having pending test results.
